Connexus PCN
Connexus Primary Care Network brings together five GP Practices serving 50,000 patients in the Brislington, St Annes, Knowle, Totterdown and Stockwood areas of South Bristol.
Connexus have Social Prescribers, Mental Health Practitioners and Mental Health coaches who work from St Anne’s House for seeing NHS patients around the local community, and who are connected to our associated GP Primary Care Practices.
